Peer-reviewed literature published from 2017 through 2026 was organized according to intervention target, relational level, communication mechanism, measurement approach, contextual modifier, and inferential strength. Clinician-focused, patient-focused, and dyadic interventions were synthesized separately before examining cross-cutting outcomes. Because the retained public search interfaces did not permit recovery of complete export-level retrieval and deduplication histories, numerical PRISMA counts were not reconstructed. Communication interventions can modify clinician skills, observable interaction, question-asking, agenda setting, preference elicitation, and perceived SDM. Effects on trust, therapeutic relationship, satisfaction, self-management, quality of life, and healthcare utilization are less uniform. Patient preferences, baseline mistrust, health literacy, communication accessibility, disease context, care-partner involvement, digital mediation, and organizational opportunity can alter intervention uptake and interpretation. Patient-centered communication interventions should be evaluated as multilevel relational interventions rather than as a homogeneous intervention class with a single outcome. Communication behavior, participation, SDM, trust, therapeutic relationship, experience, and distal outcomes require separate measurement and appropriately bounded interpretation.
