%X Health decisions are commonly represented as products of individual knowledge, preferences, beliefs, intentions, or risk appraisal. Such representations are useful but incomplete because patient action unfolds within social relationships, material constraints, healthcare organizations, information environments, and changing illness states. This conceptual article develops a proposed behavioral architecture in which beliefs and expectations remain important but acquire behavioral significance through interactions with social authorization, relational trust, practical opportunity, and healthcare-system conditions. The model distinguishes preference from enacted action, information availability from usable information, service availability from practical accessibility, interpersonal trust from institutional trust, and intended behavior from behavior sustained through learning and routine. It further treats healthcare encounters as active behavioral environments capable of modifying appraisal, perceived legitimacy, opportunity, and future participation. The framework is intentionally non-deterministic: similar beliefs can produce different actions under different material, relational, or organizational conditions, while similar behaviors can arise through different combinations of constraint and preference. The resulting architecture is proposed as a testable theoretical structure rather than an empirically validated model. It is designed to support more context-sensitive explanations of patient action and to identify where behavioral interventions may fail when they target cognition while leaving social, material, digital, or institutional conditions unchanged.
