Abstract

Medication incident reporting provides community pharmacies with opportunities to identify unsafe practices, yet the organizational arrangements required to convert local reports into shared preventive action remain insufficiently specified. Independently operated pharmacies may contribute information to common reporting systems while retaining separate authority over staffing, workflow modification, professional supervision, and implementation. This article develops a conceptual organizational learning architecture connecting incident interpretation, transferable safety knowledge, interorganizational feedback, responsibility allocation, local corrective action, and verification. A critical synthesis of community pharmacy safety research, incident investigation, organizational learning, and healthcare feedback literature informs the architecture. The analysis distinguishes reporting activity from signal quality, knowledge dissemination from accepted responsibility, and documented action from verified implementation. It proposes a distributed arrangement in which network-level functions support interpretation and coordination while individual pharmacies retain explicit responsibility for locally authorized changes. Feedback ownership provides continuity across these organizational interfaces, supported by confidentiality safeguards, documented handoffs, and escalation of unresolved tasks. The architecture also preserves recurring incident patterns and unsuccessful interventions as information for subsequent learning. Its contribution is an operationally interpretable account of how independently governed pharmacies could coordinate preventive action without assuming that reporting, feedback receipt, or administrative closure demonstrates reduced patient harm. Empirical evaluation remains necessary to determine its feasibility, implementation fidelity, and safety consequences.
